This is only possible, if you run two instances of exim on two ports
(25 and 26 for example), the one on 25 will forward all remote mail to 
26 and this one will do the rewrite and delivery to the smarthost.

Check the EXIM FAQ (maybe in exim-doc, but on www.exim.org for sure),
where this is described in more detail.
