Ah, got it solved here.  You need this in your squid.conf:

auth_param basic program /usr/lib/squid/pam_auth
auth_param basic children 5
auth_param basic realm Adzapping Squid
auth_param basic credentialsttl 24 hours

The auth_param basic realm can be changed to whatever you want; mine 
just happens to be an adzapping squid.
