So have you tried changing from VESA to SVGA? Or even VGA?

Also, have you tried cycling through the available resolutions with 
Ctrl-Alt-PlusOnTheNumberPad or Ctrl-Alt-MinusOnTheNumberPad?

Also, you might try dropping the horizontal and vertical values; 
dropping shouldn't hurt your monitor, and can sometimes force X 4.x to 
use a different configuration that can produce different results. It's 
counter-intuitive, but it's worth a shot.
