The problem is indeed Cairo, which creates a full-page transparency layer even for small images. This happens also very often when printing PDFs with evince, as evince re-renders the output with Cairo instead of passing the input PDF through.
There are already upstream bug reports on Cairo. Perhaps you should comment there. This bug report should be moved to Cairo.
