Probably the parameter of "reallocated sector counts" (initially 100 and currently 001) decreased in the last weeks till the pre-fail threshold (50) and then below: this can explain the hd behaviour you experienced.
You can configure smartmontools to self test weekly and send you an e-mail when a parameters goes down near thresholds. It isn't a 100% solution to hd troubles but it helps a lot.
