same experiences here on my machine.
after this fd0 isn't usable 'cause:

open: Device or resource busy

that's true after an attempt with superformat or mformat,
but before it works this way: 'eject fd0'
besides, I remember the same problem in '98 and never
used floppies since that time except a hfs one with
a rescue vmlinux.coff; I think formatting never worked
on pmac.
