I tried again last night with the current CVS of madwifi and even though the HAL version number hadn't changed (0.9.14.9) my B2 card now works :)
I can now highly recommend the D-Link DWL-AG650 as a safe, cheap, a/b/g cardbus card.
It recognizes APs in range and associates fine but I haven't done any link tests with the card yet.
