I would first try to contact upstream and ask him to reupload the clean package. If by any reason upstream doesn't want to, or whatever, I would repackage the .orig.tar.gz, add a get-orig-src to the debian/rules and add a note on the readme.Debian. The dsfg prefix is used when you remove files from the upstream package because they can't be redistributed or some similar case which would either make the package go to non-free or not even possible to be distributed by Debian.
