I just created, but that doesn't seem to work either.

This is for ubuntu live iso:

menuentry "Ubuntu LTS" {
    icon EFI/BOOT/icons/os_ubuntu.png
    volume 67F11057-F4C1-4504-82EC-B6EE2A558B9F
    loader   /EFI/BOOT/grubx64.efi
}

And this is for Debian:

menuentry "Debian Gnome" {
    icon EFI/BOOT/icons/os_debian.png
    volume 3AE69E07-679F-486A-8F99-338013F9A4B9
    loader   /EFI/boot/grubx64.efi
}

The only difference is in the loader path where Ubuntu uses capital 
BOOT and Debian lowercase boot. Ubuntu works well, Debian switches to 
the last boot entry in the rEFInd boot menu..

Does Ubuntu build their live images in some unusual way (or maybe 
Debian does)?

Also I tested Linux Mint, and it works well too. So it looks only 
Debian ISOs are affected.
