I think it is not good that your modem is listed as well. Did you already try to blacklist the module of the modem? I think we had cases in the past where this was necessary to get ALSA going. Have a look at /etc/modprobe.d/alsa-base-blacklist; it already has example entries for some modems. One of them is for Intel, so maybe all you have to do is uncomment that line and reboot.
