Hi, We have recently encountered a similar problem with HP LTO5 drives deployed in a StorageTek SL8500 library. Tapes to which we had previously written labels for use by our backup software (TSM) are now unreadable, even when using dd. The kernel errors reported are nearly identical to those you report.
You posted your problem two years ago, so I'm hopeful you found a solution. Any insight you can provide will be much appreciated by me and others here.
