In those cases I repeat the same steps but remove the module after bringing the interface down, then reload it. Though I'm not sure that really accomplishes anything. I don't have it as bad as the original reporter -- I can often use the connection for an hour or more with no problem. But it's guaranteed that it will happen eventually. Anecdotally it seems to be worse on 802.11n networks than it is on 802.11g. It's an Intel 5300 card in a Thinkpad X200T. I haven't used it with a kernel prior to 2.6.30 so I don't have that point of comparison.
