This sounds like an issue with pulseaudio's flat-volumes. Try disabling flat-volumes to see if it fixes it. I opened a bug report long ago to address this issue, but no decision was made [1]. [1] https://bugs.debian.org/cgi-bin/bugreport.cgi?bug=674935
