do you have gpm running? If so, does the mouse behave correctly
on the console?
I had a similar problem until I configured gpm correctly, I believe.
Not sure anymore.
My gpm.conf has
device=/dev/psaux
type=imps2
and XF86Config-4 has
Option "Protocol" "MouseManPlusPS/2"
Option "Device" "/dev/psaux"
Option "ZAxisMapping" "4 5"
