I have seen this as well (on a SuSE 7.3 machine update with XFree86 4.2 and KDE 3.0.2, so it is not Debian specific). It seems to only happen if kscreensaver attempts to start while I have VMware in full screen mode on a different virutal console. One of the symptoms of the problem is that a kdesktop_lock process is running without the screensaver running. If I "killall kdesktop_lock", the screensaver will start working again.
