This seems like a good opportunity to announce that *finally* after years of debates,¹ I can release my book from 2005 under a CC licence or somesuch. 10 years was a long time and the book is out-of-date in many places, but there are still parts of it that apply and could be ported. ¹) those were in part responsible for my lack of motivation to publish a new edition.
