Saw a post on linux-lvm mailing list that this sort of problem could be caused by device version mismatch between kernel and userspace versions of libdevmapper.
As well as libdevmapper1.02 installed, I also had (now removed) libdevmapper1.00 and libdevmapper1.01 (not currently removed - just to check on what it was doing - will remove shortly) installed.
