I've taken a look at this and found that the problem is an empty fo:inline element with an id attribute. In the DocBook 5 example, substituting the anchor element with a phrase element (keeping the element empty) causes the same problem, since the DocBook XSL-NS stylesheets translate this to the same thing. The error does not occur if there is content as a child of the element. Note, however, that anchor is defined as empty, so this does break the use of anchor.
