ath9k_htc needs a non-free firmware, which is packaged in 
firmware-atheros. Contrary to your ipw2200 attempts, 

Aug  2 07:53:55 check-missing-firmware: installing firmware package 
/media/firmware/firmware-ipw2x00_0.33_all.deb

I don't see d-i loading that firmware package from USB - are you sure 
that it's present?
