This http://www.fedora.is/CentOS/5.6/isos/i386/CentOS-5.6-i386-LiveCD.iso image
contains the LiveOS/squashfs.img file. It is a squashfs version3 image.

$ /usr/bin/unsquashfs LiveOS/squashfs.img
...
FATAL ERROR aborting: failed to read fragment table

$ file LiveOS/squashfs.img
LiveOS/squashfs.img: Squashfs filesystem, little endian, version 3.0, 719613955 
bytes, 3 inodes, blocksize: 65536 bytes, created: Wed Mar 30 15:25:05 2011

The issue can be found in the Red Hat bugtracker, 
https://bugzilla.redhat.com/show_bug.cgi?id=523504,
it was fixed upstream with 
http://squashfs.cvs.sourceforge.net/viewvc/squashfs/squashfs/squashfs-tools/unsquash-3.c?r1=1.10&r2=1.11,
rebuilding the debian package with this change is fixing the unpacking for me.
