I can't reproduce this issue here.

Do you use custom script in /etc/init.d to tell NM to sleep/wake on shutdown?

What is the content of
/var/lib/NetworkManager/NetworkManager.state
after shutdown (boot from a live cd to find out).

What is the content of this file, after boot, before you use nm-applet.

What is the output of nm-tool?

The output of NetworkManager --no-daemon would be helpful too.
Before you re-eneable networking via nm-applet, run (as root)
/etc/init.d/network-manager stop
NetworkManager --no-daemon

then check nm-applet and re-enable the network (if necessary).

Attach the log of NM please.
