While trying to create a testcase for another bug, I came across some
invalid XSL-FO generated by the XSL-NS stylesheets.  When running:

  xsltproc -o test-book.fo 
/usr/share/xml/docbook/stylesheet/docbook-xsl-ns/fo/docbook.xsl test-book.xml

invalid FO is output.  FOP complains loudly that:

  org.apache.fop.fo.ValidationException: 
"{http://www.w3.org/1999/XSL/Format}page-sequence"; is not a valid child of 
"fo:flow"! (See position 173:268)

and this is in fact the case.  fo:page-sequence is not permitted within
fo:flow.
