Usualy a FAILED message also means the script returns an error, which in turn means insserv stops executing depending scripts in that runlevel and skips to the next. Looking closer at the script I see now that mountall.sh does not report errors through its exit code. So it just makes it impossible to see if something real failed to mount because proc always fails to mount. Sorry for the severity.
