My Xen install on a VPS did not have 1777 permissions on /tmp, which seems to have prevented the root user being added.
Also I edited /etc/fstab: tmpfs /dev/shm tmpfs rw,nosuid,nodev 0 0 which previously had "default" permissions. Before any of this I did get the "debian-sys-maint" user. -- Regards, Martin Nicholas.
