I think there are two changes needed: building the kernel image needs to fail (and the old kernel left in place) if the new image would be unbootable, and d-i needs to remain working afterwards. It's not d-i's problem alone -- when I stumbled over this, it happened on a system that had been running for several years (that is, upgrading my system made it unbootable).
