When I try to upgrade from 5.0 to 5.1, I got error log in syslog like this: Installation of system tables failed! Examine the logs in /var/lib/mysql for more information. After checking postinst script, I found that it is triggered by following lines: bash /usr/bin/mysql_install_db --rpm 2>&1 | $ERR_LOGGER It is trying to run mysql_install_db even the mysql system database already exist.
