A similar bug was reported some time ago on the pkg-fso-maint mailing list. It was related to having multiple links for this application in ~/.e/e/appshadow. Could you check if this is the case for you? If so, removing one the links should make the duplicate icon disappear instantly. However, it will probably come back on next restart.
