When you start Iceweasel, does it bring up an error message saying
something like "There is an iceweasel process running but it is not

No that's not the problem. I haven't tried the other things you've suggested, but I can report that logging out and back in again isn't sufficient.

Mike Hommey has already provided a workaround "set ICEWEASEL_DSP to none in /etc/iceweasel/iceweaselrc."
