Interesting use case. If I would have been in your shoes I would probably
have tried to solve this problem with a script which creates upper case
symlinks.
Then I have removed everything else, most of them path of debian-med,
including the aforementioned dcmtk.

is the console. Reinstalling the dcmtk package makes it work again.
Thanks for your research on this. I just checked in the packaging and
are currently building the package. I wonder whether we should try
to convince RM to move the fixed package to Lenny.
