I just started seeing this on a lenny system. I suspect that it was triggered by an upgrade of cryptsetup yesterday to 2:1.0.6-7. When I boot into linux-image-2.6.26-1-686 (2.6.26-12), my system hangs with the following:
Enter passphrase to unlock the disk /dev/sda5 (sda5_crypt): <passphrase> key slot 0 unlocked. Command successful. cryptsetup: failed to setup lvm device done. Begin: Waiting for root file system ... Generating a new initrd file by running dpkg-reconfigure on cryptsetup didn't help. Booting into the kernel that I still have installed from etch, linux-image-2.6.18-6-686 (linux-image-2.6.18.dfsg.1-23etch1), still works. Here is some info when using the old kernel; please let me know if there's anything else that I can supply.
