Package: libpam-modules
Version: 1.0.1-3
Followup-For: Bug #496620

I will confirm this bug report. I have the same profile:
1. correct permissions on /etc/shadow and /sbin/unix_chkpwd
# ls -al /etc/shadow /sbin/unix_chkpwd
-rw-r----- 1 root shadow 1185 2008-08-25 06:38 /etc/shadow
-rwxr-sr-x 1 root shadow 26372 2008-08-20 14:24 /sbin/unix_chkpwd

2. screensaver unlocks correctly, but the error message is in the log:
unix_chkpwd[xxxx]: password check failed for user
