I notice several of these entries in your log:

19-Mar 12:00 alfa.domowa.pl-sd JobId 1671: 3307 Issuing autochanger "unload
slot 4, drive 0" command.

Also I noticed:

19-Mar 11:35 alfa.domowa.pl-sd JobId 1669: 3305 Autochanger "load slot 4,
drive 0", status is OK.
19-Mar 11:35 alfa.domowa.pl-sd JobId 1669: 3301 Issuing autochanger "loaded?
drive 0" command.
19-Mar 11:35 alfa.domowa.pl-sd JobId 1669: 3302 Autochanger "loaded? drive
0", result is Slot 4.
19-Mar 11:35 alfa.domowa.pl-sd JobId 1669: Warning: Director wanted
Volume "TAPE-08".
Current Volume "TAPE-04" not acceptable because:
1998 Volume "TAPE-04" status is Full, but should be Append, Purged or
Recycle.

It appears that:

* Your tape slots have different tapes than Bacula thinks they do

* Your autochanger script may be broken

I think it would be prudent to investigate these errors before pursuing the
crashing SD, as the crash may be an intentional result of this.

Please consult a Bacula mailing list if you need help resolving the issues
with your autochanger.
