I'd like to verify that your exchange box is actually talking EHLO/STARTTLS here. Can you stop your exim daemon and try having your exchange send to an exim started on the command line with exim4 -d -bd? Do other ESMTP/STARTTLS clients (such as swaks) work against your exim?
