I have such setup: the network (with real IPs) in university have outgoing 25 port being blocked. But this doesn't prevent me to have fully working e-mail sending/delivery system. That is because outgoing mail smarthost (relay) from the local net permits relaying. While doing this it also does virus, spam, other checking as it *should* be done. Configuring exim4 package for smarthost is as easy as anything else with debconf UI. And it is stated there, that ISP have similar setups, that's why smarthost menu presents. That can only be, if network have no way to send an ordinary e-mail (not just blocking 25), which is very unusual.
