You're getting a page fault during quota execution. Could you run quota with debugging enabled so we can see where it segfaults? Also, could you run a different kernel? I'd like to figure out if this is a quota bug or even a kernel problem. Since this message seem to be x86_64 specific other archs probably won't have the problem although the quota sources are the same.
