IME, 'fixed' is usually used when a bug is... fixed in an NMU, until the NMU is acknowledged by the maintainer. He does that in an upload where he should close the bug in the changelog if he thinks the bug was really fixed by the NMU. And according to http://www.debian.org/Bugs/Developer#tags: fixed This bug is fixed or worked around (by a non-maintainer upload, for example), but there's still an issue that needs to be resolved.
