Additional detail: There are choices displayed, but only *after* user presses return to the prompt What would you like to do about php.ini? Somehow the messages come in wrong order. The 1) 2) 3) ... choices should appear first and then the question "php.ini?". However, this isn't happening in Emacs M-x shell.
