I see. When the installation was made, the device was /dev/md0 which is suddenly /dev/md/0 in the initramfs boot stage without any symlinks. It seems likely that this is actually bug #382263. Could you please try updating mdadm with the experimental deb package provided in that bug report, rebuilding the initramfs image and rebooting?
