I just had the same problem even after updating the package (and
debian.cnf) and it seems, you forgot the patch (for mysql_upgrade)
provided by Erik Johansson in
http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=365433;msg=69. After
applying this patch, I do not longer get the "Got a failure from
command ..." message. This probably also solves the report by Andreas
Pakulat in http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=366155.

PS: But checking the MYSQL tables now needs a very long time, but CPU
usage does not increase to values higher than 5%. Is this normal?
