Hi Martijn,

I believe this bug has been fixed in the experimental 1.9.x series of
gnucash.

The main difference is that gnucash now prevents the user from following
the steps you describe to reproduce the bug:
To reproduce:

- Select File -> New File
- Accept all defaults (keep hitting Next until done); no accounts will be
  created.

Accepting all defaults now creates a set of Common Accounts that most
users will need. Sub-accounts can be easily added (and renamed or
deleted) from this account tree. De-selecting Common Accounts disables
the Forward button until some accounts are selected for creation. i.e.
gnucash now prevents any user creating a New file that has absolutely no
accounts.
