I ran fuser -a on /dev/snd/* after stopping pulseaudio,so it appears something in the configuration is now conflicting. default.pa worked fine for over a year.
Reverted to using custom profile to select between headphone and speaker (hoping the issue with mono audio has been resolved since I tried last time).
