This is probably a libcapi20-2 bug. Could you please provide a log of the communication? I think that capi4hylafax returns an error even if the fax is correctly sent, so hylafax reschedul it. To create this log, just use 0xfff as value for SessionTracing and ServerTracing in your /etc/hylafax/config and /etc/hylafax.* files, then restart hylafax.
