Thanks; I've now committed the changes. I'll probably upload later
this weekend after some more testing.

I have seen this when
- The dm_snapshot module is not inserted in the kernel (it doesn't get
autoloaded, which IIRC is a long-standing bug)
- Some of the existing snapshot LVs (or the source LV) are not active
(try lvchange).
BTW, your other suggestion is filed as #354257; I'm working on that
now.
