What version of gnupg are you running when this happens?  What version
of pinentry is configured (or is the default) on your system?

I've tested this with "gpg --gen-key" in a clean GNUPGHOME, using each
of pinentry-gnome3, pinentry-qt, and pinentry-gtk-2, and i have not been
able to replicate the problem.  They all show a dual passphrase entry
prompt upon key creation.
