The problem with delayed screen lock and failure to grab the mouse occurs when activating any tray icon in an lxpanel application launch bar that executes xscreensaver-command -lock from a desktop entry.
All other methods for activating the screen lock work correctly, including: Locking screen from the logout menu, locking from the lock screen item in the applications menu, locking from command line with xscreensaver-command -lock, locking screen by binding xscreensaver-command -lock to a keyboard combination in openbox. Immediately prior to testing, xdotool key "XF86LogGrabInfo" reveals that the mouse is not being grabbed by any other program. If I click the screen lock icon twice, it will lock after a delay and fail to grab mouse. Then upon unlocking the screen, it will immediately lock again and successfully grab the mouse. It seems that lxpanel or the application launch bar plugin somehow prevents xscreensaver from grabbing the mouse.
