I'll try to add as much information as I can.
Also included details about the initramfs-tools, I think it might help.

I took the machine with Debian Lenny (5.0.1) installed. It already had
raid 1 configured and working fine.
After upgrading to debian 7, it was missing /dev/mapper/{my raid}. I
noticed it because nagios client reported. 
Before updating the machine, "/dev/mapper/ddf1_RAID1-JABUTI1" was
mounted as / but now it is
"/dev/disk/by-uuid/e7a4f5f9-97d9-4e72-b5f0-b14ff55cbba1"

And   
# ls /dev/mapper/
control

I also noticed the message "modprobe: module dm-raid45 not found in
modules.dep" during the boot, just after the grub menu.
Then with some research, I discovered that the module isn't present in
the current kernels.
Then I switched "force_load dm-raid45" by "force_load dm-raid" in
/usr/share/initramfs-tools/hooks/dmraid.
But a very curious thing happened, after a reboot, all changes that I
have made was reverted to previous state. I really don't know what
happened.
Maybe after the reboot it started with the second disk as root?


Is my system missing some driver after the upgrade?
The computer is an HP Proliant DL320 G4 server.

PS: The grub information added bellow isn't accurate, because it is from
the old grub (0.97) and after the upgrade it is being used to call grub
2.
There is a option in grub legacy to finish the upgrade to grub2 but
isn't working.
