It seems that your mediawiki plugin was lacking the uploading enabling config option. The cronjob that creates the target dir which holds the wiki's attachments acts only if the [mediawiki] enable_uploads setting is true. So in your case, it failed. I'll make sure to add a check for that option in the dialog so that the admins can notify the sysadmin and ask for config option change. Hope this helps.
