Thanks for your investigations how to solve this in TeX. The attached patch [1] should integrate your solution into dblatex. Please note that it's based on the current version 0.3.4-1, stable is only allowed to get security fixes. The only location where the difference can be observed is in the document structure on the left side of the xpdf window, right? [1]
