Starting a gpg-agent via .xsession or something similar is ok. I have to start it in a vt, where pinentry doesn't work correctly. I can't send signed messages, because there is no chance to tell the gpg-agent my passphrase. For me I did not really understand the use of gpgme in relation to mutt-ng. What are the advantages and how to use them?
