quik-installer was missing a debian/copyright file. It was written by cjwatwon and has had possibly copyrightable modifications made by me and holger.
I suggest using GPL2+ for this package, since that's probably the most common license in d-i. I place my contributions to it under that license. Could the listed people please respond with a similar statement? -- see shy jo
